Podiatry · Adherence
The reason nothing worked isn’t the brace. It’s the hours.
Every corrector on the market asks for thirty minutes of your day. After twenty years of watching patients fail at exactly that, I moved the hours to the only eight nobody has to find.
Nachtopname, hoge hoek. Een voet onder een omgeslagen laken, de brace zichtbaar, de wijzerplaat vangt het enige licht in de kamer. Wekker net buiten focus op 6:02. Geen studiowit.
Ask a patient how long they wore their splint last week and they will tell you the truth as they remember it. Then look at the splint. The strap is still stiff. The dial has never been turned past the first click. It has been in the drawer since the second week, next to the toe spacers and the gel sleeves and the tape.
I am not blaming anyone. I did the same thing with a night guard for my jaw for most of a year. The problem is not willpower. The problem is that we keep asking people to find thirty minutes a day, forever, for a device that only does anything while it is on the foot.
“It only works while I’m wearing it.”
The most common objection in the category, and the honest one
That objection is correct. It is also the whole design problem, and almost nobody in this industry has treated it as one. We have spent fifteen years iterating on hinges and dials and microcurrent, and almost none on the only question that decides whether any of it matters: will this still be on the foot at six in the morning?
The afternoon I stopped prescribing daytime splints
First, what this does not do
It will not reshape the bone. Nothing you can buy will. A bunion is a change in the joint itself, and a strap on the outside of your foot does not undo it while you sleep. Anyone selling you correction of the deformity is selling you something that is, at best, unproven.
What a brace can do is hold the toe in a position it will not hold on its own, for as many hours as you can stand to have it there, which is why the hours are the entire argument.
So we built for the night
Nightform is a hinged brace with a dial on the outside of the joint and a wide instep strap that anchors behind the heel. Not a new mechanism. The hinge and the dial are the same category everyone else is in.
What is different is what we optimised for. Not degrees of correction on a chart. Three things you can check yourself, on night one:
-
The dial, not a strap you re-tighten. You set the tension once with one hand, in the dark, without sitting up. Every click holds where you left it.
-
The heel anchor. The reason a splint ends up around your ankle by 3am is that nothing behind the foot is holding it. The strap wraps the heel, so the brace has somewhere to stay.
-
Hours you already have. No thirty minutes to schedule, no reminder to ignore. You are already lying down for eight hours tonight.
